Phases of distraction osteogenesis:

  1. Osteotomy/corticotomy phase
    • Divides the bone into two segments: triggers process of angiogenesis, fibrogenesis, osteogenesis.
  2. Latency phase
    • 5-7 days. Period from bone division to onset of distraction. Inflammation and soft callus formation, which lasts 2-3 weeks.
  3. Distraction phase
    • Application of traction forces to osteotomized segments. 1mm/day for 1-3 weeks
  4. Consolidation phase
    • From cessation of traction forces to removal of distractor; newly formed bone mineralizes and increases in density strength. Lasts 3-4 months.
  5. Remodeling phase
    • From removal of disractor to application of functional loading; formation of lamellar bone
